I think melding the achievements into the actual ingame universe would be pretty cool.
In FO the achievements had no part or mention in the game at all.(That I know of, but I'm pretty sure on this one)Achievements had their own window and were always in the game, never being really introduced, but always simply there. Having characters and the universe acknowledge the presence of something like 'achievements' would add more immersion into the game and make the players feel more like they are their character, as opposed to being a person in front of a computer manipulating an avatar.
The only games I've ever played that did this were the Assassin's Creed series; where most of the achievements were presented as a list of challenges on each of one of the 3 or 4 guild's headquarters. They still functioned exactly like achievements, but it's small stuff like that little bit of acknowledgement that elevated the AC series and other AAA games over the rest of their competitors.
